REGISTRATION & FEES

To register for the congress please use the On-line Registration and Hotel Reservation Form:

Alternatively, you can download the Registration and Hotel Reservation Form and then print, complete and return the form to the Congress Secretariat ("Congress-Or") by regular mail or by fax. The deadline for the early registration is July 20^th, 2004. Delegates will be officially registered upon receipt of the Registration and Hotel Reservation Form and the appropriate Congress dues. A confirmation will be sent either by regular mail or by e-mail. Please check this confirmation very carefully to see if all items you have requested and paid for have been mentioned.

REGISTRATION FEES

• Admission to all plenary sessions, workshops, poster sessions and exhibition,

• Congress Badge, 

• Congress Bag, 

• Abstract Book, 

• Lunches on 9^th, 10^th and Farewell Lunch on 11^th of September and refreshments during breaks,

• Opening Ceremony and Welcome Reception on September 8th, 2004 at Porczynski Gallery
  (1 Bankowy Square, Warsaw) at 17:00.

• Admission to the Congress venue,

• Congress Badge,

• Lunches on 9^th, 10^th and Farewell Lunch on 11^th of September and refreshments during breaks,

• Opening Ceremony and Welcome Reception on September 8th, 2004 at Porczynski Gallery
  (1 Bankowy Square, Warsaw) at 17:00

Notification of cancellation must be made in writing to the Congress Secretariat ("Congress-Or").

• Cancellation before July 20^th, 2004 - a refund of 100% will be made.

• Cancellation before August 10^th, 2004 - a refund of  70% will be made. 

• Cancellation after August 10^th, 2004 - no refund. 

The refund will be made after the Congress.

Delegates are kindly requested to register at the Central Reception located in the Congress venue at the Institute of Biocybernetics and Biomedical Engineering, at the beginning of the Congress. Participants will be able to collect their badge and Congress documentation. Central Reception will be open at the following times:

• Wednesday, September 8^th 12:00 - 17:00

• Thursday, September 9^th 07:30 - 19:30

• Friday, September 10^th 08:00 - 18:00

• Saturday, September 11^th 08:00 - 16:00

Admission to scientific sessions and other events is permitted only to those wearing the official Congress badge.

PAYMENT

• ensure that the bank charges are borne by the participant,
• make sure that your name appears on the payment,
• send the copy of bank transfer receipt to the Congress Secretariat ("Congress-Or") either by fax or by regular mail.

Credit card payment can be made using: Visa, Diners Club, American Express, Master Card / Euro Card or JCB card. 

• make sure that the Registration and Hotel Reservation Form contains all information necessary to carry out the payment (i.e. card number and expiry date, name and address of the card holder) and that the form  has been signed by the credit card holder.
